My brother is 84 and is deteriorating mentally more than the last time I
posted about him . I visit him at least once a year but this year I'm
sick myself, so I was hoping to call him on a video call. He called me
as expected today because it's my birthday, and I asked to call him back
on my cell , and i made a video call but I didn't see his face. I hung
up and was very careful that I made a video call the second time...
still no face .He answered the phone each time and I could talk to him
and I thought since I made a video call, anytime he answered it would
also be a video call from his end too. I have very little experience
receiving video calls maybe once or twice . What is it that he was
supposed to do so that I could see his image as as well as he could see
mine ?

I'd like to be assured that he still looks good anyhow .

Nobody can be technically forced to accept a video call.
How your brother can accept a video call depends on the used software.
You did not say anything about it.

He has a new phone, but I didn't know the old phone either.

Next time I'll try to get his wife to help, but she wasn't home today

The easiest case is if both devices use the same software (Skype,
WhatsApp, Facetime whatever) that is video-capable.
